If sponsoring at one of the levels below please leave the ticket numbers blank here and write names of individuals for tickets in the space provided below after selecting sponsorship level. 

We will publish a keepsake book for the honorees and for the shul, containing photos and greetings from well-wishers. 

Sponsorships --- there are four sponsorship levels, as follows:

Platinum = $5400. This level entitles you to:

• have your greeting placed on the Inside or Back Cover of the Commemorative Book;
• a Shabbat Lunch with the Millners on a date to be determined,
• a reserved table with 10 seats to use as you see fit,
• a dedicated slide in the event's slide show, and recognition in all event communications. 

Gold = $3600. This level entitles you to:

• a full page greeting in the Commemorative Book;
• a Shabbat Lunch with the Millners on a date to be determined,
• 6 celebration tickets to use as you see fit, a dedicated slide in the event's slide show,
• and recognition in all event communications. 

Silver = $1800, This level entitles you to:

• a full page greeting in the Commemorative Book;  
• 4 celebration tickets to use as you see fit,
• a dedicated half-slide in the event's slide show,
• and recognition in all event communications.

Bronze = $1000, This level entitles you to:

• a half-page greeting in the Commemorative Book; 
• 2 celebration tickets to use as you see fit,
• and recognition in the event's slide show and all event communications. 

Donations of any amount: You have the option to simply pledge an amount that
is meaningful to you.
