Photo Authorization

Photo AuthorizationI, the parent or legal guardian of the child/ren listed below, grant Congregation Tiferet Israel permission to use photographs taken during Hebrew School or other CTI sponsored events for any legal use, including but not limited to: publicity, copyright purposes, illustration, advertising, social edia, and web content. Furthermore, I understand that no royalty, fee, or other compensation shall become payable to me by reason of such use.
